A Photo Release Form is used when people are photographed, and the photographer (the owner of the photo) wants to publish the images.

During the course of RUF events & programs, lots of photos are taken, and we need to be able to publish some of these photos. As such, we need to obtain a signed photo release form from you, the subject of the photo (or, in the case of a minor, the parent or guardian).
